Questions people ask

Why must I open a write-up before I can sign it?

Signing says you have read it. The list shows a summary, but you sign only after opening the whole account.

More in “Review and countersign session write-ups”
What happens when I send one back?

The mentor is told your reason and can change the same write-up and send it again.

More in “Review and countersign session write-ups”
Where are plans I need to sign?

On the other tab of the same Sign-off queue. See the guide on signing plans.

More in “Review and countersign session write-ups”
Can I sign a write-up I wrote?

No. Another manager signs it.

More in “Review and countersign session write-ups”
How do I find write-ups nobody has written?

Use the Write-ups needing attention list. It shows planned sessions with no write-up.

More in “Review and countersign session write-ups”
Can a signed write-up be corrected?

Yes, by a manager or admin. They can amend it or correct the delivery, and the history is kept.

More in “Review and countersign session write-ups”
Where do I start a write-up?

From the session in the Planner, with Record session. It opens the form with the plan's details filled in.

More in “Write up a session”
Does Save & exit send my write-up?

No. Save & exit keeps a draft. Submit for sign-off sends it.

More in “Write up a session”
Can I write up a session that did not happen?

Yes, with a short note: the outcome, why, and whether you saw the young person. See the guide on recording one.

More in “Write up a session”
Who reads my write-up?

A manager reads it and signs it, or sends it back with what to change.

More in “Write up a session”
Is there a guide for each part of the form?

Yes. One covers the times and ratings, one the account in your own words, and one raising a concern and sending it.

More in “Write up a session”
What if the form says there is already a write-up?

It is advice. Open the one listed if it is the same session, or carry on if it was a second one.

More in “Write up a session”
Why is the finish time empty?

It is empty on purpose, so the record shows when the session really ended and not when it was planned to. Type it before you send.

More in “Write-up: the times, the outcome and the ratings”
Can I change the start time that came from the plan?

Yes. The times are yours to correct on the write-up. The plan itself locks once recording starts.

More in “Write-up: the times, the outcome and the ratings”
What is the difference between Delivered and Delivered (late)?

Delivered (late) is for a session that did happen, but later than planned. Both count as delivered and ask for the full write-up.

More in “Write-up: the times, the outcome and the ratings”
Why can't I see the ratings?

They only show for a delivered session. For any other outcome you give a reason instead.

More in “Write-up: the times, the outcome and the ratings”
What do the numbers on the ratings mean?

1 is Not yet, 2 is With prompt, 3 is Most of the time and 4 is Transferring. Choose the one that matches what you saw.

More in “Write-up: the times, the outcome and the ratings”
Do I have to rate a session I am writing up late?

No. After the day it happened the ratings are optional. Leave them blank rather than guess.

More in “Write-up: the times, the outcome and the ratings”
Why does the form ask where it happened?

The plan did not say. Choose one of the listed venues, or describe the place. A delivered session cannot be sent without it.

More in “Write-up: the times, the outcome and the ratings”
Do I have to fill in every box?

For a delivered session, the summary, the young person's words, positives and reflection are needed before it can be sent. Evidence is only a prompt.

More in “Write-up: say what happened in your own words”
What goes in Actions taken?

What you did: the support, the changes you made and what was agreed next. The summary says what happened.

More in “Write-up: say what happened in your own words”
Can I use dictation?

Yes, with Dictate session, if your organisation allows it. Read the text it proposes and correct it before you use it.

More in “Write-up: say what happened in your own words”
What if the young person said nothing?

Say so in Pupil voice, in a sentence. An honest short answer is better than an invented quote.

More in “Write-up: say what happened in your own words”
Is the reflection seen by anyone?

Your manager reads it when they sign off the write-up.

More in “Write-up: say what happened in your own words”
Why does the box stop accepting text?

Each box has a limit. When it is full the form tells you, and you can carry on in another box.

More in “Write-up: say what happened in your own words”
Do I need a photo every time?

No. The form asks once if there is none and lets you send without. Plenty of sessions have nothing to photograph.

More in “Write-up: say what happened in your own words”
What if I choose Something to raise but close the concern form?

The write-up cannot be sent until the concern is recorded. Open it again with Record it now and finish it, or choose Nothing to report if that is right.

More in “Write-up: raise a concern and send it off”
Does the concern go to the DSL when I send the write-up?

It is recorded when you save the concern form, and the DSL can see it then. The message after you send confirms it.

More in “Write-up: raise a concern and send it off”
Why does it say my draft is saved but I cannot send?

Some sections still need finishing. The message lists them. Press a name to jump to that box.

More in “Write-up: raise a concern and send it off”
What is Submit anyway?

It sends a write-up that has an empty box the form only advises on, such as evidence. It does not skip sections that must be done.

More in “Write-up: raise a concern and send it off”
Why am I asked why my write-up is late?

Your organisation asks for a short reason after the day. You can type one or send without. It is never refused.

More in “Write-up: raise a concern and send it off”
Can I send it back to myself to edit?

Not once it is sent. Ask your manager to return it. It then shows as Resubmit for sign-off when you open it.

More in “Write-up: raise a concern and send it off”
Where does my write-up go after I send it?

To a manager to read and countersign. They either sign it or send it back with the changes they want.

More in “Write-up: raise a concern and send it off”
Do I still write up a session that did not happen?

Yes, but only a short note: the outcome, a category, why, and whether you saw the young person. No ratings are needed.

More in “Record a session that did not happen”
What counts as safe and well?

You saw the young person that day. Choose Yes, I saw them and the count of missed sessions starts again from zero.

More in “Record a session that did not happen”
What is the difference between No show and Refused?

No show means nobody turned up. Refused means the young person declined to take part. They are reported separately.

More in “Record a session that did not happen”
Why can't I pick a category?

No show and Service constraint have no list. Use the reason box to explain what happened.

More in “Record a session that did not happen”
Can a manager change the outcome later?

Yes. A manager can correct the delivery outcome on a signed-off write-up. They must give a reason.

More in “Record a session that did not happen”
What if I answered Not delivered by mistake on Home?

There is no undo on Home. Tell your manager what happened so they can correct it.

More in “Record a session that did not happen”
Why does it say this young person already has a write-up today?

There is a write-up on file for them for that day. If it is your session, open it. If it was a second one, press Carry on with this write-up.

More in “Why it says there is already a write-up”
Can I write two write-ups for one day?

Yes, when there really were two sessions. The warning is advice only. If sending is refused, the message names the record it clashes with.

More in “Why it says there is already a write-up”
I pressed Record and landed on an old write-up. Why?

That session already has one. This session is already written up. tells you so. A second blank form would not reach the same record.

More in “Why it says there is already a write-up”
How do I change a write-up I have already sent?

You cannot edit it. Ask a manager to send it back, or use Ask for it back when the form offers it.

More in “Why it says there is already a write-up”
What does Open the existing record do?

It opens the record in a new tab, so what you have typed here is kept.

More in “Why it says there is already a write-up”
Will I lose what I have typed?

No. Your draft is saved, and nothing is lost when you open the other record.

More in “Why it says there is already a write-up”
How do I know a write-up was sent back?

You get a message that begins with the reason. The record also shows Sent back to you, and the Planner offers Make requested changes.

More in “Fix a write-up that was sent back”
Can I start a new write-up instead?

No. Fix the returned one. A new one would leave it stranded, and the form warns you if you try.

More in “Fix a write-up that was sent back”
I changed it, but my manager has not seen it. Why?

You probably saved a draft. Press Resubmit for sign-off to send it.

More in “Fix a write-up that was sent back”
Where is the reason they gave?

In the message you were sent, and under Sign-off chain at the foot of the record.

More in “Fix a write-up that was sent back”
Who can fix a returned write-up?

The mentor who wrote it. A colleague can read it, but it is not theirs to change.

More in “Fix a write-up that was sent back”
What if I disagree with the changes asked for?

Talk to the manager who returned it. If you resubmit unchanged, say why in the account so they can see.

More in “Fix a write-up that was sent back”
Why can't I edit a write-up I sent?

Once it is sent it is held for the manager to read, so it cannot move underneath them. Ask them to return it, then use Make the requested changes.

More in “Read a write-up, and correct a signed one”
What is the difference between Amend and Correct delivery?

Amend opens the whole account. Correct delivery changes only whether it happened and why.

More in “Read a write-up, and correct a signed one”
Who can amend a signed-off write-up?

Managers and admins.

More in “Read a write-up, and correct a signed one”
Does amending erase the original?

No. The history is kept, and the status shows Amended.

More in “Read a write-up, and correct a signed one”
What does Awaiting manager mean?

It has been sent and nobody has signed it yet. It shows in the countersign queue.

More in “Read a write-up, and correct a signed one”
Where do I see who signed it?

Under Sign-off chain at the foot of the page, with the date and any reason given.

More in “Read a write-up, and correct a signed one”
Why is a session on this list?

It was planned, its day has passed, and no write-up exists for it. Open it to see who it belongs to.

More in “Find the write-ups nobody has written”
Can a mentor still write it up late?

Yes. A write-up is never locked for being late. They may be asked for a short reason when they send it.

More in “Find the write-ups nobody has written”
Why have some rows disappeared when I filter?

The last chip hides accounts with no sessions in 28 days. A note says how many it hid. Press Show them to see them.

More in “Find the write-ups nobody has written”
Does a session marked Not delivered show here?

No. Answering Not delivered writes a note, so it is not outstanding.

More in “Find the write-ups nobody has written”
What does Open write-up do?

It opens the write-up form for that session, so you or the mentor can see what is needed. It does not send anything.

More in “Find the write-ups nobody has written”
Where do I turn the late reason on or off?

Press Change this in Settings in the note near the top, if you have permission. If not, the note says who can.

More in “Find the write-ups nobody has written”
What is the difference between signing a plan and signing a write-up?

A plan is signed before the visit and says the preparation is sound. A write-up is signed after, and says the account is complete. They sit on separate tabs of the same queue.

More in “Sign off the plans people send you”
Why can't I sign a plan I wrote?

Someone else has to check it. The row says your own plan is signed by another manager.

More in “Sign off the plans people send you”
What does the mentor see when I send a plan back?

A message with your reason as its first line. They change the plan and send it again.

More in “Sign off the plans people send you”
Can I sign many plans at once?

Yes. Tick them and press Sign off selected. Read each plan first, because signing says you have.

More in “Sign off the plans people send you”
Why is the planning tab empty?

Nothing is waiting, or your list is narrower than the service's. Check the choice that shows everyone's queue.

More in “Sign off the plans people send you”
Does signing a plan stop the mentor changing it?

No. They can still change it until recording starts.

More in “Sign off the plans people send you”
Can I stop mentors having to write pupil voice?

Yes. Choose Optional for that card, or untick Shown. You can also reword it to suit you.

More in “Change what the write-up form asks for”
Can I start with a completely empty form?

Yes. Press Start from a blank form. Everything that can be hidden is hidden. Attendance and welfare stay, because they carry the safeguarding. Then add your own questions.

More in “Change what the write-up form asks for”
Can I add a score out of 10?

Yes. Press + Add your own question, choose Score, and set how long it runs, from 2 to 10, with words at each end.

No. Attendance and welfare are always shown and always required. You can reword them.

More in “Change what the write-up form asks for”
What is the difference between Mandatory and Ask once?

More in “Change what the write-up form asks for”
How do I go back to how it was?

Press Use our standard form, then Save. Your own questions are kept.

More in “Change what the write-up form asks for”
Does this change write-ups already sent?

No. It changes the form from now on, and old write-ups read back in the words they were written in.
