Write-up: raise a concern and send it off

Answer the welfare question, deal with anything the form says is missing, and send the write-up for sign-off.

Checked against the screens:

Every organisation can choose its own words for the people and roles it works with. When you are signed in, this guide uses your organisation’s words. Otherwise it uses the product’s own.

Watch the last part: safeguarding and sending it off.

For the mentor finishing a write-up. If a child is in danger now, follow your safeguarding procedure and speak to the DSL first. A write-up can wait.

Steps

  1. Find the card Welfare & safeguarding. It asks whether anything in the session worried you.
  2. If not, choose Nothing to report. If so, choose Something to raise and press Record it now to fill in the concern form.
  3. Finish the concern before you go on. Closing the form without recording it stops the write-up being sent.
  4. Press Submit for sign-off. After a return, the button reads Resubmit for sign-off.
  5. If a box says One section needs finishing, or lists several, press a name to jump to it and fill it in.
  6. If the form says some boxes are empty, read them. Press Submit anyway if you have nothing to add.
  7. Check the message that confirms it is sent. If you raised a concern, you may be offered Add the full account.

If it goes wrong

  • Your draft is always saved. Nothing is lost when the form stops you. Use Save & exit if you must leave.
  • The message says the concern could not be recorded? The write-up is saved. Tell your DSL directly today.
  • Asked why it is late? Type a short reason and press Save with this reason. Or press Save without a reason.
  • The page says it was already submitted? Nothing more is needed. Your manager can send it back if it needs changing.

The welfare question

This card is always shown and always required. Nothing to report is a complete answer. Something to raise opens the concern form, which records the concern and links it to this session.

The write-up only checks that you finished what you started. If you chose Something to raise and did not record it, the form says so. You can finish it, or change your answer.

What the form checks before it sends

Some sections must be finished: the outcome, the ratings, what happened, the words, positives, welfare and reflection. A delivered session also needs a finish time and a place. Where the plan did not name a place, the form asks.

Other prompts are advice. A missing photo, or goals not linked on an unplanned session, are asked once and then let through with Submit anyway.

Sending late

A write-up is never locked for being late. If your organisation asks for a reason, a short box appears when you send. You can type a reason or send without one. Either way it is saved and marked late.

After you send

The write-up goes to a manager to countersign. It then shows as awaiting a manager in the Planner. If you raised a concern, your DSL can see it at once. You can add the full account of what was said and done now or later.

If two people write the same one

When a co-delivering colleague edits the same draft, the form shows what changed and lets you keep your words or theirs before you send. Nothing is overwritten without a choice.

Sending a write-upWelfare comes first. A concern is raised separately from the write-up.
  1. WelfareNothing to report, or raise it
  2. CheckedMissing sections listed

  4. CountersignedOr sent back

How it links together

A concern recorded from the welfare card goes to the safeguarding queue, where the DSL sees it, and is linked back to this session. The write-up itself goes to the countersign queue and changes the Planner label to awaiting a manager. A late write-up is marked late for managers. See the guide to raising a concern and the guide to a returned write-up.

Questions people ask

What if I choose Something to raise but close the concern form?

The write-up cannot be sent until the concern is recorded. Open it again with Record it now and finish it, or choose Nothing to report if that is right.

Does the concern go to the DSL when I send the write-up?

It is recorded when you save the concern form, and the DSL can see it then. The message after you send confirms it.

Why does it say my draft is saved but I cannot send?

Some sections still need finishing. The message lists them. Press a name to jump to that box.

What is Submit anyway?

It sends a write-up that has an empty box the form only advises on, such as evidence. It does not skip sections that must be done.

Why am I asked why my write-up is late?

Your organisation asks for a short reason after the day. You can type one or send without. It is never refused.

Can I send it back to myself to edit?

Not once it is sent. Ask your manager to return it. It then shows as Resubmit for sign-off when you open it.

Where does my write-up go after I send it?

To a manager to read and countersign. They either sign it or send it back with the changes they want.
