Watch the second part: times, outcome and ratings.
For the mentor writing up a session. Open it from the Planner with Record session. The form saves as you go.
Steps
- At the top, check the strip headed Write-up for the planned session. For a session nobody booked, it reads Unplanned session.
- Check Pickup time and Start time. They are filled in from the plan. Correct them if the real times were different.
- Type the real Finish time. It starts empty on purpose, so the record shows when it really ended. Times use the 24-hour clock.
- Under Attendance & outcome, choose the Outcome. Most are Delivered.
- If it did not go ahead, choose a Category, explain why, and say whether you saw them.
- If it was delivered, open Rate the session and score Engagement, Achievement and Goal progress from 1 to 4.
- Carry on to the next cards, or press Save & exit to come back to a saved draft.
If it goes wrong
- A note under the times says the finish is before the start? Check you used the 24-hour clock. The note offers the likely answer.
- No ratings on screen? They only appear for a delivered session.
- Writing up after the day? The ratings and the young person's words are optional. Leave them blank rather than guess.
Planned times and real times
The plan says what you intended. The write-up says what happened. The date and place stay with the plan, but the times are yours to correct, because the plan locks once recording starts.
Do not change the times to make a clash warning go away. If two sessions overlap, tell your manager.
The outcomes
- Delivered: it happened as planned.
- Delivered (late): it happened, later than planned.
- Cancelled by the young person, or by the mentor.
- No show: nobody came.
- Refused: the young person would not take part.
- Service constraint: the service could not run it.
Every outcome except delivered asks for a category, a short reason, and whether you saw the young person that day. See the guide on recording a session that did not happen.
Where it happened
If the plan never said where, a short question asks for the venue or a description of the place. A delivered session cannot be sent without it. An unplanned write-up has the full Where & when card with date, times, location type and venue.
The three ratings
Each rating is 1 to 4: Not yet, With prompt, Most of the time or Transferring. Engagement is how present they were. Achievement is how well they met the aims. Goal progress is progress on the linked goals.
Ratings feed reports that track progress, so they should agree with what you write. A late write-up lets you leave them blank, and they are recorded as not captured at the time.
Drafts
Your answers save as you work. Save & exit leaves a draft you can open again from the Planner. Saving is not sending. Sending is covered in the guide on sending a write-up off.
How it links together
The times start from the plan and are corrected here. The outcome feeds attendance and the count of missed sessions, and the ratings feed progress reports. The record page, the countersign queue and the Planner label all read what you enter. See the guide to the written account and the guide to sending a write-up for the rest of the form.
