Every session that has a write-up has a record page. Anyone with access to the young person can read it. Only the right person sees a button to change it.
The status at the top
- Awaiting the author: a draft. Its author can carry on, with Finish this write-up.
- Sent back to you: a manager returned it. Its author presses Make the requested changes.
- Awaiting manager: it is sent and waiting to be countersigned. It cannot be edited.
- Signed off: countersigned. Only a manager can change it.
- Amended: a manager changed it after it was signed.
The sections
Below the header you see the ratings, Linked goals, then the account: Aims, What happened, Positive observations, Pupil voice, Actions taken and Reflective practice. A pill reading Welfare concern raised means the session was linked to a concern. Sign-off chain lists who signed or returned it.
If it goes wrong
- No edit button? A sent write-up cannot be edited by its author. Ask a manager to return it.
- Need to change a signed write-up? Only a manager or admin can, with Amend.
- Only the outcome is wrong? Use Correct delivery instead of amending the whole account.
Amend a signed write-up
Amend opens the same form as the original, with Save amendment in place of the send button. The amendment is recorded against the manager who made it, and the status becomes Amended. The earlier history is kept.
Correct the delivery only
Correct delivery changes whether the session happened, why, and whether the young person was seen. It does not touch the written account. A window called Correct delivery outcome asks for the new outcome and a reason for the correction. Press Save correction.
Attendance and the count of missed sessions update to match, and the correction is recorded with your reason.
Who sees which button
- The author of an unsent or returned write-up: the button to carry on writing.
- A co-delivering colleague: they share the draft until it is sent.
- Managers and admins: Amend and Correct delivery on a signed write-up.
- Everyone else: the page to read, with no buttons.
Evidence and safeguarding
Photos and files attached to the session appear at the foot of the page. If the session was flagged for safeguarding attention, the page says so and points to the concern record in the young person's safeguarding section.
Arriving here from Record
If you pressed Record on a session that was already written up, you are brought here. A note says so, and explains what to do next.

How it links together
This page is the record that the Planner, the countersign queue, the young person's sessions tab and reports all point to. Amending or correcting the delivery updates attendance and the count of missed sessions, and keeps the history. See the guide to a returned write-up and the guide to statuses.
