Find your way around Questionnaires

See your templates, make your own copy of a standard one, and find the routine checks and invitations waiting for you.

Checked against the screens:

Every organisation can choose its own words for the people and roles it works with. When you are signed in, this guide uses your organisation’s words. Otherwise it uses the product’s own.

Questionnaires is for managers and admins. Anyone else sees a message that says a manager or admin is needed.

Steps

  1. Open Questionnaires. The tabs across the top are Overview, Parent feedback, Responses, Templates & appearance and Onboarding packs.
  2. Stay on Templates & appearance. Look under Routine checks to do first. Press Record check on any that are due.
  3. Find a form under Templates, grouped as Feedback, Consent, Incident or Checklist. A Default badge means the standard version.
  4. To change a standard form, press Customise. You get your own copy. Later, press Edit your version. To undo, press Revert to default.
  5. To use a form, press Send on it, or Fill in if it is a checklist.
  6. Press New template to make one from scratch. Press Form themes to change how forms look.

If it goes wrong

  • If you see a message that you cannot change templates, your organisation limits who can. Ask an admin.
  • If Customise or Revert to default fails, nothing was changed. The message says why. Try again.
  • Recent invitations shows the last few sent. If one is missing, open the Responses tab for the full list.

The workspace tabs

  • Overview: scores, trends and the send button. See the guide on insights.
  • Parent feedback: the form families receive after a report, and the replies.
  • Responses: every invitation and answer.
  • Templates & appearance: this page.
  • Onboarding packs: the welcome pack for new families.

Standard forms and your own copies

A form with a Default badge belongs to the platform and is shared by every organisation. You cannot edit it. Customise makes your organisation's own copy and opens it ready to edit. The standard form stays untouched. A copy is marked Your version.

Revert to default takes your copy out of use and goes back to the standard form.

Routine checks to do

Templates live here. Sending creates invitations, answers return to the Responses tab, and insights summarise them. Routine checks to do come from checklist forms and the venues you look after. Answers about a child also appear on that child's record.

Questions people ask

Why can't I open Questionnaires?

It needs the manager or admin role. Ask an admin if you think you should have it.

What is the difference between a standard form and my version?

A standard form is shared by all organisations and cannot be edited. Your version is a copy only your organisation uses, and you can edit it.

Can I get the standard form back?

Yes. Press Revert to default. Your copy is taken out of use and the standard form is used again.

How do I send a questionnaire?

Press Send on the form. A window opens to choose who it goes to. See the guide on sending a questionnaire.

Why do some forms say Fill in instead of Send?

Those are checklists. You complete them yourself at a venue. You do not send them to anyone.

Where do I find the answers?

Open the Responses tab, or press Insights for scores and trends.
