Send a questionnaire and share its link

Choose a form and a recipient, create the invitation, then email it or share the link or QR code.

Checked against the screens:

Every organisation can choose its own words for the people and roles it works with. When you are signed in, this guide uses your organisation’s words. Otherwise it uses the product’s own.

Sending is for managers and admins. The old Send page now opens the Questionnaires overview, where the same window is built in.

Steps

  1. On Questionnaires, open the Overview and press Send a questionnaire. A window opens.
  2. Choose the Template. Drafts are not offered, because only published forms can be sent.
  3. Choose who it is about and who gets it: the young person and one of their contacts, or type an email address and name.
  4. Check Expires in (days). After this many days the link stops working.
  5. Press Create invitation. If an address was given, the email is sent now.
  6. On Invitation created, press Copy link or View QR code to share it yourself, or Send another.

If it goes wrong

  • If the email did not send, the invitation still exists and the link works. Copy the link and send it yourself.
  • If it says to pick a template or a young person, that form needs one. Choose and try again.
  • If a feedback form is about a team member, pick that person when asked. It is required for those forms.

Who it is for

Most forms are about a young person. You choose the young person and then one of their contacts, such as a parent. Their name and email fill in. Organisation-wide checklists and forms for colleagues are not tied to a young person, and the window says so.

No email address on file

If a contact has no email, type an email address yourself, or leave it blank. With no email, nothing is sent. You get the link to share another way. You can also type a display name for the recipient.

Feedback about a team member

Some forms collect feedback about one mentor. For those, the window asks which mentor the feedback is about and requires an answer. On other forms the box is optional and only used to group feedback per person.

The link and QR code

Each invitation has its own link. View QR code opens a picture you can print or show on a screen. Treat each link as private to its recipient. It stops working after the number of days you chose.

Sending from other places

Pressing Send on a form on the Templates tab opens this window with that form chosen. A link that names a young person opens it with them already chosen.

After sending

The new invitation appears in the Responses tab as pending. It changes to opened when someone views it and to submitted when they finish.

How it links together

What sending sets goingSending creates an invitation that you can follow to the end.
  1. ChooseForm and recipient
  2. InviteEmail or link
  3. AnswerThey fill it in
  4. ReadResponses tab

The invitation records the form version, the recipient and the expiry. If the email goes, the recipient gets a link. When they submit, the answer binds to the child and shows on the record, and insights update. Nobody on your team is told automatically, so check the Responses tab.
