Only managers and admins can edit forms. A standard shared form cannot be edited: press Customise on the Questionnaires page to make your own copy first.
Steps
- Open a form with Edit on the Questionnaires page. You land on the Questions tab.
- Press / or the add question button. Choose a type from the list, then type the question and any help text.
- Choose the Question type, tick Required if it must be answered, and add choices where the type needs them. Drag a row to reorder it.
- Open Intro / Outro to write the welcome and thank-you text. Open Theme to choose how the form looks.
- Check Versions to see what changed. The form saves as you go.
- Press Publish and confirm. To stop using it, press Move back to draft.
If it goes wrong
- If an error banner appears, read it. It says what could not be saved. Change that item and the form saves again.
- Use the undo and redo arrows if you changed the wrong thing.
- A published form that has answers should be changed with care. Check Versions first.
Question types
You can use short and long text, yes or no, single choice, multi-select, image choice, and four kinds of scale: emoji, star, thumb and trophy. An info block adds a heading or a paragraph, and a signature type asks for a signature. Some types are marked as not ready, so avoid them for real forms.
Help text and simpler wording
Help text appears under a question. For children under eleven you can add a simpler version of each question. The Simplify button suggests one with AI. Read the suggestion before you accept it, because you are responsible for the final wording.
Conditions and hidden questions
A condition shows a question only when an earlier yes or no, or single-choice, answer matches. Add a yes or no question first. Tick the hidden box to stop a question being asked without deleting it. For a yes or no question you can also show a tell us more box after the answer.
Intro and outro, theme and sender
Intro / Outro holds a cover image, the introduction, the thank-you text and the sender name and reply-to address. A form using the safeguarding theme has a fixed thank-you message that you cannot change. The Theme tab chooses a theme for this form only.
Versions and the snippet library
Every save that changes the form adds a version, and the Versions tab shows what was added or changed. Snippet library holds questions you saved to reuse. Select a question and save it as a snippet.
Publishing
A draft cannot be sent. Publishing makes the form available immediately. For your own copy of a standard form, a message says it replaces the built-in form for everyone in your organisation from then on. Switch back to the built-in form at any time.
How it links together
- DraftEdit freely
- PublishOffered for sending
- SentInvitations use this version
- VersionsHistory is kept
Each invitation remembers the version of the form it was sent with, so earlier answers still match their questions. The theme, introduction and sender details shape what recipients see and the emails they get. Results flow to the Responses tab and to insights. See the guide on themes.
