Create a new questionnaire template

Start a blank form of your own, choose its type and who it is for, then go on to add the questions.

Checked against the screens:

Every organisation can choose its own words for the people and roles it works with. When you are signed in, this guide uses your organisation’s words. Otherwise it uses the product’s own.

Only managers and admins can create templates. If your organisation limits who can change forms, a message tells you.

Steps

  1. On Questionnaires, press New template. The page is called New questionnaire template.
  2. Type a Slug (unique, kebab or snake case). It is the form's short code, such as year_end_review. You cannot use one twice.
  3. Type a Title. Add a short description if it helps your team.
  4. Choose a Category and an Audience. Tick Requires e-signature if the person must sign.
  5. Tick the box to allow anonymous answers if the form need not be tied to a young person.
  6. Press Create template (draft). The editor opens with an empty draft.

If it goes wrong

  • If a message says a slug and a title are both needed, fill both in. Nothing was saved.
  • If the slug is already in use, the message says so. Choose another one.
  • Your draft is not visible to anyone you send forms to until you publish it.

The slug

The slug is a short code that identifies the form. Use lower-case words joined by underscores or hyphens. It is used in web addresses and filters, so choose it carefully. Pick a clear title for people, and keep the slug for the system.

Category

  • Feedback: gathers views from the person you send it to.
  • Consent: records agreement.
  • Incident: records what happened.
  • Checklist: a routine check completed by staff at a venue.

Audience

The audience says who answers. Choose the young person, a parent or guardian, a stakeholder, a colleague, or internal staff. It changes who you can pick when you send the form.

Signature and anonymous answers

Requires e-signature adds a signature step at the end. Allow anonymous means the form does not have to be tied to a young person. Use it for surveys, not for records you need to keep on a file.

What happens next

You land in the editor with a draft. Add questions, choose a theme, write an introduction, then publish. A draft cannot be sent.

How it links together

From blank form to answersA new template is the first step of the cycle.
  1. CreateSlug and type
  2. EditAdd questions
  3. PublishNow sendable
  4. SendTo recipients

The category and audience you choose decide how the form is used: a checklist is filled in by staff, other forms are sent to people. A draft appears only in the templates list. After publishing, it is offered when you send, and its answers feed insights.

Questions people ask

Can I change the slug later?

The slug is set when you create the form. If you chose badly, make a new template and use that.

Why can't I create a template?

You need the manager or admin role. Your organisation may also limit who can change forms. Ask an admin.

Is the new form sent to anyone?

No. It starts as a draft. Nobody can answer it until you publish and send it.

What does the audience change?

It says who the form is for, such as a parent or a colleague. It decides who you can pick when you send it.

Can I turn a standard form into my own?

Yes. On the Questionnaires page, press Customise on the standard form to make your own copy.
