Record an accident or incident

File an accident record or a staff incident report, with a signature, so a pattern can be spotted later.

Checked against the screens:

Every organisation can choose its own words for the people and roles it works with. When you are signed in, this guide uses your organisation’s words. Otherwise it uses the product’s own.

For anyone who saw a slip, trip or injury. A worry about a child's welfare goes through a concern instead. If a child is in immediate danger, call 999 first, then phone your DSL. Do not wait for the app.

Steps

  1. Open the hub and press Record an accident.
  2. Choose the young person it happened to. Leave it empty if no child was hurt.
  3. Under Where? (optional), pick the place if it is on the list.
  4. Answer every question marked as needed. Say what happened, in order.
  5. If the form asks for a signature, tick I confirm the statement above..
  6. Type your own name in Type your full name to sign.
  7. Press Record incident. You will see Incident recorded.
  8. Press Record another for a second one, or Back to Safeguarding.

If it goes wrong

  • If a required question is empty, the form says which one. Fill it in and press again.
  • If you see a note about an unfinished report on this device, restore it and finish it.
  • If it will not save, write down the facts and phone your manager. Try again later.
  • If the accident also raises a welfare worry, raise a concern as well. The two records are separate.

How it links together

A filed incident shows on the safeguarding hub's recent list and, when you chose a young person, on their record. Managers and the DSL can read it. If the event also worries you about welfare, raise a concern as well; see the guide on raising one.

What this form is for

The accident record and the safeguarding report are forms your organisation can word for itself. They capture what happened, who was there and what was done.

They are for events, not for a child telling you something worrying. For that, raise a concern, because a concern reaches the DSL queue.

Who can use it

Anyone signed in can file one. The hub also lists recent incidents so a manager can spot a pattern, such as repeated falls at one place.

The signature

Some forms need a signature. You type your full name and tick the confirmation. This shows who stands behind the statement. Do not sign for someone else.

Choosing the place

If the incident happened at one of your venues, choose it. That lets a manager see which venue has the problem. If the list is empty, leave it and describe the place in your answers.

After you file it

The record is kept with the young person when you chose one. A manager or DSL can read it. You cannot edit it afterwards, so read it before you press the button.

If you left something out, tell your manager. They can arrange for it to be added.

Questions people ask

Is an accident the same as a concern?

No. An accident record is for injuries and slips. A concern is for worries about welfare or safety. Some events need both.

Do I need to choose a young person?

Only if a child was involved. For a staff injury or a visitor, leave it empty.

Why do I have to sign it?

The signature shows who stands behind the account. Type your own full name and tick the box.

Can I edit it after I press Record incident?

No. Tell your manager what was missed so it can be added.

Who can read what I record?

Your manager and the DSL can. Other staff see only what their role allows.

What if I lose signal?

Your answers are kept on the device. Open the form again and restore them. Phone your manager if it is urgent.
