Add a risk assessment and its hazards

Start an assessment on a record, add hazards, get it approved, close hazards and begin a new review when it is due.

Checked against the screens:

Every organisation can choose its own words for the people and roles it works with. When you are signed in, this guide uses your organisation’s words. Otherwise it uses the product’s own.

Risks sit in the Risk tab of a record, in the risk register. Mentors, managers and admins can add. Only managers and admins can approve.

Steps

  1. Open the record and choose the Risk tab. If there is no assessment yet, press Add an assessment.
  2. Choose the kind, tick the starting hazards you want, and press Start the assessment.
  3. To add your own, press Add a hazard. Describe it, say how serious it is, and press Add the hazard.
  4. Ask a manager to press Approve cycle once the controls are right. An approved assessment is locked.
  5. When a hazard is dealt with, press Close on its row, then confirm Close item.
  6. When the next check is due, press Start a new review. Confirm with Start the new review.
  7. Check the next review date and that the badge reads Approved.

If it goes wrong

  • If a save cannot be confirmed, reload before trying again. If the hazard is listed, it was saved.
  • An approved assessment cannot be edited. Start a new review to change it.
  • A hazard that is also a worry about harm needs a concern as well. Raise one separately.

How it links together

Risks are read before a visit, so a hazard here shows to the people planning and delivering. An overdue review appears on the record. Concerns about harm are a separate guide.

The register on the record

The register holds one current assessment for each kind. Each shows its cycle number and when it was reviewed. It shows when the next review is due and how many hazards it has. It also shows whether it is Approved or Pending approval. A badge warns when a review is overdue or due soon.

A fold-out called Questions to think about gives prompts for that kind of assessment. Read them before you write the hazards.

Adding a hazard

A hazard needs a few words describing it. Choose how to rate it: one overall word, or likelihood and severity. A hazard linked to a health need or a diet shows that link, so it is not lost.

The fuller form

A longer form exists at the address for a new risk on the record. It asks for the Hazard, who is at risk, how they might be harmed, the Existing controls, the Further actions and a Review due date. You rate it with scores from one to five, or with one overall word.

An owner can be chosen with Choose owner. Leave it alone and it uses your own account. Nothing in the ordinary pages links to this form, so use the register buttons unless you were given the address.

Approval, review and closing

Approving fixes the assessment for that cycle. After that, recording that an action was done and adding a comment are still allowed on each hazard. Anything else needs a new review.

Closing a hazard says it has been dealt with. Do it only when something was actually done. Mark reviewed today is a separate step, described in the guide to reading a risk assessment.

Old links to a single risk

An old link to one risk now takes you to the register on the record. Nothing is lost. The register is where every risk lives.

Questions people ask

Who can add a risk assessment?

Mentors, managers and admins. Only managers and admins can approve one.

Why can't I edit an approved assessment?

Approval locks it for that cycle. Start a new review to change it. You can still record an action as done and add comments.

What does Pending approval mean?

The assessment has been written but nobody authorised has approved it yet. Ask a manager to approve it.

What is a new review?

It starts the next cycle of the same assessment, so you can update controls and have them approved again.

I followed an old link and landed on the register. Why?

Single risk pages were retired. The register on the record holds every risk, so you are sent there.

Does closing a hazard delete it?

No. It stays on the register as closed, so the history is kept.
