Check record quality and fix the gaps

See who has a current risk assessment, learning plan, wellbeing plan and passport, and open the records that need one.

Checked against the screens:

Every organisation can choose its own words for the people and roles it works with. When you are signed in, this guide uses your organisation’s words. Otherwise it uses the product’s own.

These four pages are for managers and admins. Anyone else is sent back to Home.

Steps

  1. Open Record quality. It starts on Risk-assessment coverage. Use the links across the top to switch between the four checks.
  2. Read the lines above the table. They say how many are shown and how many on this page have a current plan.
  3. Look down the last column for No current readable. That young person has a gap.
  4. Press the name to open the record at the right place, then add or update the document there.
  5. Press Next to read the next page, and Previous to go back. The counts describe one page at a time.
  6. Come back later and check the page again. A new document turns the row to Current.

If it goes wrong

  • If the page says it could not be loaded, reload it before you rely on it. A blank result is not the same as no gaps.
  • If a plan is there but the row says there is none, you may not have access to read it. Ask an admin.
  • Use the mentor audit link at the top to find which team members are behind.

The four checks

  • Risk-assessment coverage: a current readable risk assessment. It also counts those with a review past its own date.
  • ILP coverage: a current readable ILP. The page uses your organisation's own name for it.
  • Wellbeing-plan coverage: a current readable wellbeing plan.
  • Passport coverage: a current readable therapeutic passport.

Who is counted

Every young person you can read who is not archived. Active, paused and ended records are all included. The first line gives the total and how many appear on this page.

What current means

A row says Current readable when a plan exists and you may read it. It says No current readable when none exists, or when it is not one you can read. The page is clear that a plan being present does not mean it is complete or approved.

How to fix a gap

Press the name. The record opens with the right drawer ready: risk, ILP, wellbeing or passport. Add the document there. On the risk check, a row marked review overdue needs the review done, not a new assessment.

Related pages

The links at the top also lead to Moving ILPs to the platform and Consent pack returns, and to the mentor audit. Use them with this page to see whether a gap is a missing plan or a missing consent.

Why the counts are per page

The figures describe the page you are on, not the whole organisation. Add the totals across pages if you need an organisation figure, or use the reporting dashboard.

How it links together

From gap to fixThese pages show the gaps. The fix is made on the record.
  1. CheckCoverage page
  2. GapNo current plan
  3. RecordAdd the document
  4. ClearRow shows Current

Each page reads the plans stored on children's records. Adding a plan on the record changes the row here the next time you open the page. Nobody is told that a gap exists, so tell the team yourself.

Questions people ask

Why don't I see the Record quality pages?

They need the manager or admin role. Anyone else is sent to Home.

What does 'No current readable' mean?

There is no current plan you can read for that young person. Either none exists or it is out of your access.

Does a current plan mean it is good?

No. The page only shows that one exists. It does not judge whether it is complete or approved.

How do I fix a missing ILP?

Open the ILP coverage check, press the young person's name and add the plan in the ILP drawer.

Why are the counts different on each page?

Each count describes the page you are on, not the whole list.

Are ended records included?

Yes. Active, paused and ended records are all included. Archived ones are not.
